To help you find the services you need, we encourage you to visit the following Web sites: Community Partners Community Partners provides services and support to people with developmental disabilities and people with mental health concerns. A private, nonprofit agency serving citizens of Strafford County, Community Partners is one of twelve New Hampshire agencies designated by the State of New Hampshire for the delivery of community mental health and developmental services. Familes First Health & Support Center Family Resource Connection Compiled by the State of New Hampshire, this page provides links to NH Healthy Kids (free and low cost health insurance for New Hampshire children), WIC (Women, Infants & Children nutrition program), home health providers, and organizations that support children with special needs and ongoing health care needs. Healthy Kids Healthy Kids is a low-cost health insurance program for uninsured children. Its philosophy is to promote healthy lifestyles, encourage preventive health and dental care, treat illness early and manage chronic health conditions. New Hampshire Healthy Kids Corporation (NHHK) was established by special legislative act in 1993. Through partnerships with Blue Cross Blue Shield of New Hampshire, Delta Dental and the state's twenty-six hospitals, NHHK covers income-eligible children, from birth to age 19. InfoLink If you have medication needs for a chronic condition and are uninsured, you may be eligible for free prescriptions through various medication assistance programs available to you. The Medication Financial Assistance Program is a short-term medication assistance program funded by The Foundation for Seacoast Health, and designed to assist those who need short-term medications but are uninsured. This program is specifically for the residents of Eliot, York and Kittery, Maine, and residents of Greenland, New Castle, Newington, Portsmouth, North Hampton and Rye, New Hampshire. For those patients of doctors affiliated with Portsmouth Regional Hospital, who have long-term medication needs due to chronic illness but are uninsured, the Portsmouth Regional Hospital Prescription Assistance Program may be available to you. For more information, to determine your eligibility, or to find out how you can enroll contact the Program Coordinator at our InfoLink Office. InfoLink Office Portsmouth Regional Prescription Assistance Program The Portsmouth Regional Prescription Assistance Program is part of the Medication Bridge Program, which has locations throughout New Hampshire. The goal of the program is to help eligible uninsured and underinsured patients of all ages to receive needed prescription medications from pharmaceutical companies. Locally, the NH Medication Bridge Program is funded by Portsmouth Regional Hospital and its Medical Staff, with in-kind support from InfoLink.SeaCare www.seacarehealthservices.org SeaCare Health Services helps people who do not have health insurance and cannot afford to pay for medical, dental and mental health services. Doctors, nurses, therapists, dentists, chiropractors and other health care providers donate all of the care to SeaCare participants. Since 1995, SeaCare has coordinated the delivery of over $1,750,000 in medical services to families in the Seacoast. In addition to the multi-disciplinary health network, SeaCare offers other supports for families in the area. Portsmouth Regional Hospital provides ongoing financial support to SeaCare, and many of the physicians on our medical staff participate in the SeaCare health network. Seacoast Mental Health Center Seacoast Mental Health Center is private, nonprofit provider of mental health services (including 24 hour emergency services) serving the New Hampshire and southern Maine Seacoast. Its mission is to provide the highest quality of psychiatric, counseling and psychological services in a safe and confidential environment. Seacoast Mental Health Center is accredited by The Joint Commission on the Accreditation of Healthcare Organizations. |
